The nature of my work, being wholistic health and including naturopathic work, is different from just the EP therapies, but for what it's worth, you can take a look at the intake form I use. I am attaching my Personal History Information Form. I don't know if this weblist accepts email attachments. If it does, you will get it. Otherwise write to me and I'll em you seperately. Hope that helps you somewhat.
