Need Practice Advice

Hi Everyone:

     This is my first counseling practice and I am needing some advice from
all you experienced counselors out there. In my ad I am introducing and
promoting EmoTrance but I will also use EFT and my own style of creative
counseling around prosperity and weight loss issues, creative visualization
which is half trance and half Ttimeline, plus I do goal crafting, empowerment,
metaphysical work,...you get the picture.
     Anyway, I am looking for samples of general intake forms for new clients
to fill out. I want to keep things fairly simple.
     And, I need some sample agreements to look at that say things like "I'm
not a psychotherapist, I do have to report child abuse, I'm a spiritual
counselor or energy worker or metaphysical counselor, etc. and some verbage on
what to expect or not expect to be clear so they don't assume I am their
therapist. Maybe some words on fees, late and cancelled appointments, the
standard stuff.
     Would anyone be willing to help me here? What are other Energy Therapists
doing in this regard to CYA?
